Click on reinstatement form below and reinstatement instructions. Please print from a home desktop or laptop computer. Do not attempt to print from an office computer or phone due to firewalls.
You can email gail@aspt.org if you are unsure of the fees that are owed.
ANYONE THAT IS NEEDING TO REINSTATE AFTER THREE OR MORE YEARS, PLEASE CONTACT GAIL@ASPT.ORG and YOU WILL BE REQUIRED TO PURCHASE A PHLEBOTOMY or EKG MANUAL AND RETAKE THE EXAM ONLINE IF YOU ARE A FORMER MEMBER
If you need to reinstate within the three-year time limit, you must go to www.aspt.org and purchase a CEU quiz and print your reinstatement form and contact gail@aspt.org for fees owed.
