Reinstatement
Click on reinstatement form below and reinstatement instructions. Please print from a home desktop or laptop computer. Do not attempt to print from an office computer or phone due to firewalls.
You can email gail@aspt.org if you are unsure of the fees that are owed.
ASPT must receive your reinstatement no later than November 24th in the NC office. If mailing or emailing late, please add $30 late fee or it will be returned to you. Postmarks do not count as being received.
ANYONE THAT IS NEEDING TO REINSTATE AFTER THREE OR MORE YEARS, PLEASE CONTACT GAIL@ASPT.ORG and YOU WILL BE REQUIRED TO PURCHASE A PHLEBOTOMY or EKG MANUAL AND RETAKE THE EXAM ONLINE IF YOU ARE A FORMER MEMBER
If you need to reinstate within the three-year time limit, you must go to www.aspt.org and purchase a CEU quiz and print your reinstatement form and contact gail@aspt.org for fees owed.
