Regards to a waiver:

While ASPT recognizes the tenure of its senior Phlebotomists, it is the policy of the certification guidelines that all Phlebotomists desiring certification submit to the ASPT national certification, unless holding a national certification by ASCP, AMT, and NCA. All individuals requesting a waiver must submit a copy of their national certificate and grades. An application/membership fee for this waiver is $90.00. ASPT requires a completed practical exam on all applicants for certification. Waivers apply only to the CPT written exam.

Submit your application and fees and you may come to an exam location and be tested on the practical. You may also allow your supervisor to give you the exam. The guidelines for an in-house exam must be followed.

ASPT can offer an In-House certification for those areas where there is no feasible way to schedule a regular examination. Applicants must go to a scheduled exam site to be tested, if there is one within two months of the desired date, or if the examination is within 100 miles of the applicant's home. Fees must be paid 30 days in advance and must be accompanied by a letter on the institution's letterhead, and from the lab manager or supervisor. The letter must state the test monitor's qualifications and state that the monitor would be willing to give the examination, the date the exam will be given, the location where the examination will be held. The letter must also state that the examination must be given within ASPT's guidelines. Allow at least 30 days after we receive the In-House letter request to sent the test monitor all testing materials.
